No communication with mentor (could rarely book time on his calendar).
Bad workplace environment (a particular SDE 2 would often be called out during stand-up versus in a private discussion with the manager).
It should be noted that my experience was extremely subjective (the project deadline for the team lined up with when my internship was ending, so that contributed to the difficulty in communication), but it should be noted.
