The tech stack gets stale. We kept forcing the same framework/tools for every project without considering alternatives and upgrades.
Management and planning were lacking in my team. People were put in misfit roles, and projects got delayed due to everyone trying to work on the same thing (which they shouldn't be working on).
There were not a lot of growth opportunities.
