So much emphasis on coding standards and reviews, for example, but a real lack of planning for the code development in every one of the groups I worked in. Development is not just writing code, but a majority of the engineers I encountered didn’t understand that.

And Perf, the performance review system: what a monster created, albeit with good intentions. Such a massive waste of effort for the gain.
