Many years without a raise, with the explanation given being that I'd have reached my level cap. That has been the same story I've been told over the last 8 years.

Apparently, managers don't actually have any leverage to arrange for performance-related pay increases, as that seems to be entirely in the hands of HR.

If you have employees that perform at the level of only 15% of your top performers, over a period of 8 years, and those employees can't get any raises due to company-set level ranges, maybe that shouldn't be made into a problem for the high-performing employee, but instead for his executives and managers that consistently fail to promote people into higher levels.
I'm fully aware that it's much more convenient and efficient to keep people doing the same stuff forever when they are the best in their field, and only promote people that are easy to replace in their current job because they don't contribute that much anyway. But that kind of mean-spirited behavior should at least be disguised a bit more and not be rubbed into your employees' faces year over year.
