Reduce redundant groups and roles at the "working man's" level.
Certain organizations and business units have created many silo groups that do not interact efficiently.
Create a structure with clear ownership and responsibility. If you cannot see exactly who owns a process or is responsible by the organization structure, then change it so you can.
Matrix organizations do not work; they just make it impossible to have an ownership culture.
