- Awful conduct in meetings - if you don't know how to shout, you won't get heard.
- Bad management decisions regarding which technologies to use.
- The actual use of some technologies was poorly executed, resulting in many problems that revealed themselves over the years.
A serious cultural makeover is necessary.
