Internal politics are rampant and cause too much unnecessary churn in projects. The wrong people are often chosen to lead projects, and they typically ignore the right people. Too many managers are trying to be technical.
Stamp out internal politics and empire building.
Really try to implement a collaborative culture instead of allowing many to just pay lip service to it.
