Bad management. Unrealistic pressures and expectations. Groupism. Manager maintains a pet group under him. If you are not in the group, no matter what good work you do, it goes unnoticed. Customers are too touchy and literally blackmail openly about cancelling orders if some trivial bug is not fixed within time. This shakes up the whole management structure from director to apply pressure and get work done, no matter what. It's ruthless. Software tools are open source and very difficult to debug, as debug infrastructure is not taken seriously. No hikes at all. Promotions only if you are in the above-mentioned pet group.
Get rid of the old boys' groups in Mgmt and get some new, good guys.
Shake up the pet group.
