Sub-par management team, especially on the engineering side. It seems that they hire management teams that are less qualified than their underlings.
Stop the madness! Just because someone held a management position at a previous company does not mean they were good at it. They could have just run the project into the ground and then blamed someone else on that team. Unfortunately, they play the blame game about three or four times until people catch on and see where the real problem is. Try to find a better method of recruiting management, instead of creating this endless cycle of failure.
