Promotions are given out based on whether you're liked and how you compare to others. They have nothing to do with how well you do your job or if you exceed your targets.
Pay rises aren't great. Little to no investment in training.
Invest more in the staff you have instead of managing them out the door and replacing them with someone in a country with a cheaper labor market.
Take extenuating circumstances into account on reviews and judge people on the goals you make them set, not on how well they perform against others.
