Too many levels of hierarchy and many overlapping roles. The rules for advancement and success are more political rather than real. So people spend more time in discussions rather than actual work.
Don't just pretend to listen, but actually listen to the engineers from all levels.
You don't need large forums to listen to them. Have small group discussions and take action by giving formal responsibility to all levels.
