A very bureaucratic, hierarchical, risk-averse, and functionally siloed organization can often mean many meetings and dealing with significant amounts of overhead at times.

Lack of commitment and effort towards quality software practices, with shortcuts for temporary speed being the norm.

Management needs to realize that long-term speed comes from making investments in quality and good practices up front. Taking shortcuts early on in a project for the sake of speed produces some initial success, with a lot of subsequent failure.
