Too many middle managers, too much focus on nonexistent metrics polluting the actual work being done. Small changes developers turn in are shipped via email. I've seen ship mails where they changed a data type from integer to double and found some wins.
Stop the nonsense around shipped emails and "plus plus" being used as perf doc metrics.
