Crappy immediate management. If your immediate manager is sexist and elitist, you are SOL. The manager's supervisors won't question your immediate manager's treatment of you, no matter how inappropriate.
Make managers base evaluations and promotions on facts and performance, not personal biases and unfounded assumptions.
