Poor system of appraisal where people at the bottom of the hierarchy suffer a lot. Despite your managers being aware of your work, they will question it at the time of appraisal, saying that you did not do anything extra.
Focus on people doing the real work for the client, and not on useless presentations or "brown bag" sessions which people give just for the sake of giving them.
