Middle management can't seem to stick to decisions.
Very often we would start adding a feature, at management request, only to have management change their mind before or even after the feature was completed. This was bad for productivity, bad for morale, and bad for the bottom line.
Upper and middle management hiring, promotion, and layoffs are based way too much on cronyism.
Make a decision and stick with it. Hire based on knowledge, experience, and competence, not just because they're your buddy.
Reward managers who encourage productivity and creativity. Reward individual contributors who contribute.
