There is a lot of group-think and an unspoken requirement to drink the Kool-Aid. Communication doesn't always happen in the appropriate channels, so it's easy to become disconnected.
Make it less appealing for employees to move around a lot within the company. Or, rather, make it more appealing for employees to stay with their groups longer. There's so much overhead from the chaos of people moving around all the time.
